Swimming has a reputation as one of the most joint-friendly sports on the planet. It's low-impact, buoyant, and often recommended for injury recovery. Coaches, doctors, and fitness enthusiasts alike praise it as a lifelong activity that's gentle on the body.

So why do so many swimmers — from age-group competitors to weekend lap swimmers — end up with shoulder pain?

The numbers tell a sobering story: shoulder injuries are the most common overuse issue in swimming, with rotator cuff-related problems leading the list. The culprit isn't swimming itself. It's how you swim. And more specifically, it's what your shoulder blades are (or aren't) doing during those thousands of repetitive strokes.


The Impingement Trap: What's Actually Happening Inside Your Shoulder

To understand why swimmers' shoulders hurt, we need to look at a condition called Shoulder Impingement Syndrome.

Here's the anatomy in plain English: your rotator cuff tendons pass through a narrow space beneath the acromion (the bony tip of your shoulder blade). Every time you raise your arm, those tendons slide through this confined tunnel. Normally, there's just enough room for smooth, friction-free movement.

But when your shoulder blade — the scapula — fails to rotate upward and outward as your arm lifts, that space narrows. The tendons get pinched. Repetitive pinching leads to inflammation. Inflammation leads to fraying. And fraying, left unchecked, can lead to partial or even full rotator cuff tears.

In freestyle swimming, the most vulnerable phases are:

Most recreational swimmers obsess over the "pull" phase — the powerful part where water is being pushed backward. But that's not where injuries typically start. They start in the entry and recovery, where bad habits quietly accumulate over thousands of yards.


Technique Fixes That Save Shoulders

The good news: most shoulder pain in swimmers is preventable through relatively simple technique adjustments. Here are the two most impactful corrections:

Fix #1: The High-Elbow Catch

A "dropped elbow" during the catch phase forces your shoulder into internal rotation — compressing the rotator cuff against the acromion. The fix is maintaining a high elbow position where your fingertips point downward and your elbow stays higher than your wrist.

Think of reaching over a barrel, not shoveling sideways.

Fix #2: The Entry Point — Stop Crossing the Middle

Many swimmers — especially those who breathe to one side — develop a habit of crossing their hand over the centerline of their head on entry. This not only creates drag (snaking through the water) but also forces the shoulder into a vulnerable, internally rotated position at the very moment of highest impact.

Fix it by: Imagining a lane line running from your head straight down through your body. Your hand should enter at 11 o'clock and 1 o'clock relative to your head — never at 12 o'clock. And keep your thumb pointed forward or slightly upward on entry. A thumb-down entry internally rotates the shoulder before it even begins the pull.


Dry-Land Defense: Build Stability Out of the Water

The best time to protect your shoulders is when you're not swimming. A targeted dry-land routine builds the muscular support system that keeps your shoulder blade tracking correctly through every stroke phase.

Here's a simple 15-minute routine you can do anywhere — no pool required:

Perform this routine 3–4 times per week, especially on days you're not swimming. Consistency matters more than intensity.


Gear That Supports the Shoulder

While technique and strength training are the true foundation, the right equipment can make a meaningful difference — especially during high-volume training periods.

Compression garments with targeted shoulder support can provide mild compression that increases proprioception — your body's awareness of joint position. This helps you "feel" when your shoulder drifts into a vulnerable position. Look for swim tops or training shirts with seamless construction in the shoulder area to eliminate chafing during repetitive arm movement.

Training aids serve dual purposes: they build strength and provide feedback. Paddles increase resistance, forcing your pulling muscles to work harder — but use them sparingly and only with perfect form, as oversized paddles can actually aggravate impingement. Pull buoys allow you to focus on upper-body mechanics without the distraction of kicking, making it easier to concentrate on entry point and high-elbow positioning. Fins can help with body position, taking pressure off the shoulders by allowing you to maintain a higher, more streamlined posture in the water.

It's also worth understanding that not all strokes are created equal when it comes to shoulder stress. Butterfly and freestyle place the greatest demands on the rotator cuff due to the overhead recovery. Backstroke, while still demanding, places the shoulder in a more externally rotated position that many find less impingement-prone. Breaststroke is generally the kindest to shoulders — but introduces other biomechanical challenges. If you're managing a sensitive shoulder, consider adjusting your stroke mix accordingly.


Why This Matters to the American Swimmer

The United States has one of the largest swimming populations in the world. From summer league kids to high school state champions to the tens of thousands of adults participating in U.S. Masters Swimming, the pool is a familiar space for millions of Americans.

Yet high-quality, technique-focused swimming education remains surprisingly scarce. Most swim instruction stops at "how to move through water" — not "how to move through water for 50 years without breaking down." The "no pain, no gain" mentality still lingers in many lanes, especially among adult swimmers who returned to the sport later in life and are now logging serious yardage for the first time since childhood.

For this audience, a prevention-first message lands with force. Telling a Masters swimmer "how to swim faster" is useful — but telling them "how to keep swimming for decades without shoulder surgery" is urgent. It speaks directly to their deepest anxiety: that the sport they love might eventually be taken away by an injury that feels inevitable.

It's not inevitable. Not if you understand the shoulder. Not if you fix the entry. Not if you do the dry-land work. Not if you choose your gear wisely.


Protect What Carries You Through the Water

Your shoulders are the most valuable equipment you own as a swimmer. They carry you through every practice, every race, every open-water crossing. And they're remarkably resilient — if you treat them with the respect they deserve.

Start with one change: film yourself swimming a few laps and watch your hand entry. Is it crossing the centerline? Is your thumb pointing down? Is your elbow dropping before the catch?

Pick one correction. Apply it for a week. Notice the difference in how your shoulders feel after a 2,000-yard set.

Because swimming shouldn't hurt. Not like this. Not for long.
